How to Diagnose Migraine Clinical Importance of Location

Abstract
Background: It is very important to decide the localization (side and site) of pain sensitive points in headache patients. There are limited informations available on the constancy of location. In this study we clarified the tender points to decide the location of migraineurs.

Methods and Subjects: The 200 migraineurs had no other neurological or system diseases. We used headache questionnaire based on international headache classification and routine laboratory test. We palpated both side and several sites on the head with even pressure, which showed abnormal finding such as tenderness, tightness and sensitivity to pain. We decided the right or left side and sites of one side. The palpation points were periorbital, preauricular, temporal, parietal, occipital and neck.

Results: Among 200 migraineurs, 24 had pain sensitive points on the both sides, 98 had on the right side and 78 had on the left side. The painful sites were diffuse rather than localized. We could draw any lines along the tender points which corresponded meridian systems documented in eastern medicine.

Conclusion: If one can accurately localize the side and site that triggers a migraine headache, one may treat an attack efficiently with pharmacological or non-pharmacological method.
